You can download and print the forms at the links below to complete with your doctor at your next visit. Please see each health reward form for eligibility and program details.

Participants in the CalOptima Health Program of All-Inclusive Care for the Elderly (PACE) are not eligible for health rewards. Kaiser Permanente members may only participate in the COVID-19 Vaccine Member Health Rewards. Kaiser Permanente members are not eligible for other health rewards.
